A Support at Home provider is an organisation or entity that is approved and accredited by the Australian government to deliver aged care services to individuals who have been assessed as eligible to receive a Support at Home funding. These providers play a crucial role in the delivery of personalised care and support to older Australians who wish to remain living independently in their own homes. The responsibilities and services of a support at Home provider include developing a care plan; management of funds; monitoring and reviewing care, and compliance with aged care regulations and legislation.

All Support at Home packages need an assigned care manager, registered to an approved provider, to administer the government subsidy. Each provider is different, offering different types of services and/or care. It’s up to you to choose the provider that best fits your needs and the level of control you want over your budget and who delivers your services. You will need to complete an aged care assessment before you start with an aged care provider.

Self-Managed Support at Home providers

Home care is all about maintaining independence, so it makes sense that the system enables you to retain as much control as you are comfortable with. When it comes to administering your Home Care Package, a Self Managed provider allows you to tailor your care to suit your preferred levels of choice and control. This might mean that they administer your HCP subsidy, while you:

  • Choose your own support workers
  • Negotiate your services at rates you’re comfortable paying
  • Go directly to workers rather than using a middleman

Services provided under a Support at Home are subject to your personal care plan, but can include:

  • Personal care: Assistance with bathing, dressing, grooming, and mobility.

  • Domestic assistance: Help with cleaning, laundry, and meal preparation.
  • Social support: Companionship, social activities, and community engagement.
  • Transportation: Assistance with shopping, medical appointments, and social outings.
