home and community Care Program for Younger People

Home and Community Care Program for Younger People (HACC PYP) supports Victorians to optimise their health and wellbeing, while maintaining or regaining their independence to live safely in their homes, and actively participate in their community.

This may be due to chronic illness, mental health issues, disability or other conditions where they need one-off, intermittent or ongoing support to undertake the activities of daily living.

MHA Care is a not-for-profit provider and offer HACC PYP assessments and services to the Greater Shepparton and Moira Shire areas.

Our friendly and experienced support professionals are here to help you work towards your goals with care and understanding.

Victorians who meet the following criteria are eligible for HACC-PYP;

  • Under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander people)
  • At risk of losing your independence due to disability, chronic medical condition or mental health challenge
  • Not receiving NDIS or My Aged Care funding
  • Carers of a person (as above) who has difficulty carrying out the tasks of daily living
  • In-home respite
  • Domestic assistance
  • Meal assistance
  • Cooked meals assistance
  • Personal care (showering, shopping)
  • Property maintenance (e.g. gutter clean, smoke detectors)
  • Social support
  • Planned activity group
  • Capacity building activity (one off or short-term service support to help build skills to remain living independently)
  • Service linkages (including Allied Health referral

Home and Community Care is a needs-based program and eligibility does not guarantee access.

Yes, a client contribution is required. During your assessment, we will discuss your financial details to determine whether you are in the low, middle, or high income bracket. This helps us calculate the contribution you will be asked to make towards the services you receive.

Please note: there is no cost for a HACC PYP assessment itself.

  • A HACC PYP Referral Form will need to be completed
  • You can self-refer by obtaining a form in person from MHA Care or download it from our website www.mhacare.org.au
  • Ask your health professional or service provider to supply the form or complete it on your behalf (with your consent)
  • Send the completed form via email, post, or in person to the MHA Care intake team

A HACC PYP assessor will make contact for a Living at Home Assessment to assess eligibility and your support needs
