home and community Care Program for Younger People

The Home and Community Care Program for Younger People (HACC PYP) helps eligible Victorians maintain their independence and continue living safely in their own homes while staying connected to their local community.

At MHA Care, we believe in locals supporting locals. Our team understands the value of remaining in the place you call home and being part of the community you know and love. Through funding provided by the Victorian Government, we’re able to offer practical support that helps make everyday life a little easier.

HACC PYP is designed for younger people who may be experiencing challenges due to disability, chronic illness, mental health conditions, or other circumstances that make daily tasks more difficult. Support may be needed on a one-off, occasional, or ongoing basis.

Depending on your individual needs and the program guidelines, MHA Care can assist with services such as domestic assistance, social support, and other practical help around the home. Our goal is to work alongside you to support your independence, wellbeing, and ability to continue living the life you choose.

As a local provider, we’re proud to support members of our community with compassionate, reliable services that help people stay independent, connected, and living well at home.

MHA Care is a not-for-profit provider and offer HACC PYP assessments and services to the Greater Shepparton and Moira Shire areas.

Our friendly and experienced support professionals are here to help you work towards your goals with care and understanding.
